Beacon In Home Health does not exclude people or treat them less favorably on any of these bases.
Free communication and accessibility help
When needed to ensure equal access and effective communication, Beacon In Home Health provides the following services free of charge and in a timely manner.
For people with disabilities
We provide reasonable modifications and appropriate auxiliary aids and services, which may include:
- Qualified sign language interpreters
- Information in large print, audio, accessible electronic formats, or other formats
- Other reasonable changes needed for equal access to our services
For people whose primary language is not English
We provide language assistance services, which may include:
- Qualified oral interpreters
- Electronic or written translated documents
- Help understanding information about care, benefits, rights, and services
Request help or file a grievance
To request a reasonable modification, auxiliary aid or service, or language assistance, contact Graham Wood. You may also contact Graham if you believe Beacon In Home Health failed to provide these services or discriminated in another way.
A grievance may be filed in person or by mail, phone, fax, or email. If you need help preparing or filing a grievance, Graham is available to assist you. Beacon In Home Health will not retaliate against anyone for requesting assistance, filing a grievance, or participating in an investigation.
File a complaint with HHS Office for Civil Rights
You may also file a civil rights complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. Filing a grievance with Beacon In Home Health does not prevent you from filing with HHS.
