SKILLED NURSING SERVICES
Beacon In Home Health offers client centered care that enables you to maintain a quality lifestyle.
- Registered and Licensed Nurses will observe each new client to assess individual care needs and then work with the family to carry out the physician’s plan of care.
- Skilled Nurses also teach patients, families, and care givers about medication management, home safety and other issues to encourage independence.
- Medical Social Workers are available to assist with financial, social and emotional concerns to help you cope with challenges. They can connect families with community resources to address housing, food and other health issues which may impact recovery.
- Registered Dietician can assist in developing dietary compliance plans and needs.
- Home Health Aides can assist with all the activities of daily living.
- Physical Therapists works with you to restore muscle strength and flexibility.
- Occupational Therapists can help you regain independence by helping you train your body to perform daily living tasks
- Speech Therapists will help to restore communication skills and abilities such as eating and swallowing.
If services include use of the client’s personal funds:
The Agency will obtain an authorization from the client/responsible party, for access to personal funds when grocery or other shopping services are to be provided and when those services include assistance with bill paying or any activities, such as shopping, that involve access to or use of client funds.
YOUR NEEDS ARE IMPORTANT TO US
Services offered by Beacon In Home Health are available 7 days a week, 24 hours a day, in the patient’s place of residence. A supervisor is available by telephone during these hours.
CARE MANAGEMENT
We will work with you to create a customized plan of care, and manage it every step of the way. Our experienced team of care management specialists, RNs, and friendly companions who can provide transportation if needed are there to ensure the highest quality of care is consistently provided.

When routine tasks become too much to handle alone, Beacon In Home Health offers hourly services to assist our clients with daily activities such as bathing, dressing, cooking, cleaning, medication reminders, errands and much more. By helping with daily activities, our hourly home caregivers enable clients to maintain their normal routines and independence, while staying in the comfort of their home.
